New Patient Information

New patient registration is processed at the center you visit. As a new patient, we ask that you arrive at least 30 minutes prior to your appointment time. Please bring your current photo identification and health insurance card. If you are applying for our Sliding Fee Scale Discount Program, please download the appropriate form below.
